Overview

The Environmental Protection Agency (EPA) requires certified commercial applicators to record and maintain for the period of at least two years routine operational records containing information on types, amounts, uses, dates, and places of application of restricted use pesticides and for ensuring that such records will be available to appropriate State officials.

The records must contain:

  1. The name and address of the person for whom the restricted use pesticide was applied.
  2. The location of the restricted use pesticide application.
  3. The size of the area treated. This information should be recorded in the unit of measure (such as acre, linear feet, bushel, cubic feet, number of animals, etc.) which is normally expressed on the label in reference to the application being made. For example, if an 80-acre grove is treated using the alternate middle approach, the entire 80 acres would be recorded as the “size of area treated.”
  4. The time and date of the restricted use pesticide application.
  5. The brand or product name of the restricted use pesticide applied.
  6. The EPA registration number of the restricted use pesticide applied.
  7. The total amount of the restricted use pesticide applied per location per application.
  8. The name and certification number of the certified applicator that made or supervised the application, and, if applicable, the name of any noncertified applicator(s) that made the application under the direct supervision of the certified applicator.
