Overview

The Occupational Safety and Health Administration (OSHA) requires employers to prepare and maintain records of occupational injuries and illnesses. The employer must complete an injury summary on Form 300-A, or an equivalent, which shows the total number of recordable injuries and illnesses for any given year in each category.

At the end of each calendar year, employers must:

  1. Review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified;
  2. Create an annual summary of injuries and illnesses recorded on the OSHA 300 Log;
  3. Certify the summary; and
  4. Post the annual summary.

 

Employers must post a copy of the annual summary in each establishment in a conspicuous place where notices to employees are customarily posted. They must ensure that the posted annual summary is not altered, defaced or covered by other material. It must be posted no later than February 1st of the year following the year covered by the records and kept in place until April 30th. OSHA logs must be maintained on file for five years following the year to which they relate.
